The BiWaze Clear provides lung expansion, mucus mobilization, and aerosol therapy in a portable 10-minute session.
RT’s Three Key Takeaways:
- Multimodal Therapy: The system combines positive expiratory pressure, high-frequency oscillation, and nebulization into a single 10-minute session to treat atelectasis and mobilize secretions.
- Portable Design: Weighing 9 pounds and featuring an integrated battery, the device is designed for use in both acute care and home healthcare environments.
- Reimbursement Coding: The Centers for Medicare & Medicaid Services (CMS) introduced HCPCS code E0469 for lung expansion and high-frequency oscillation systems, effective October 1, 2024.
ABM Respiratory Care is providing a portable solution for airway clearance with its BiWaze Clear system, which integrates multiple respiratory therapies to treat atelectasis and mobilize secretions.
The system delivers oscillating lung expansion (OLE) therapy through a dual-blower design and a closed, dual-lumen circuit. This configuration generates an expiratory flow bias to move mucus toward the upper airways, according to ABM Respiratory Care.
“BiWaze Clear delivers efficient, precise oscillating lung expansion (OLE) therapy to re-expand collapsed airways, mobilize retained mucus deep in the lungs, and deliver aerosol, supporting lung health across both hospital and home settings,” ABM Respiratory Care reports.
The 10-minute therapy sessions alternate between positive expiratory pressure (PEP) and high-frequency oscillation (HFO) every 2.5 minutes. During these cycles, an integrated Aerogen Solo vibrating mesh nebulizer provides continuous aerosol therapy to loosen mucus.
The device weighs 9 pounds and includes a touchscreen interface and an integrated battery. These features allow for use in both acute care settings and for long-term respiratory wellness at home, according to the company. ABM Respiratory Care stated that independent testing confirmed the system provides consistent and accurate pressure delivery compared to other available systems.
The design is intended to reduce therapy time, support earlier hospital discharge, and help maintain respiratory health in domestic settings. The system is also designed to reduce the spread of aerosol particles outside the closed circuit to protect patients and healthcare providers.
Effective October 1, 2024, the Centers for Medicare & Medicaid Services (CMS) introduced HCPCS code E0469 for lung expansion airway clearance, continuous high-frequency oscillation, and nebulization systems. The agency also introduced code A7021 for the associated monthly disposables.
